STAGE READY ACADEMY

MINOR PARTICIPANT WAIVER, RELEASE, AND MEDIA CONSENT

IMPORTANT: By proceeding to registration, you acknowledge that you have read and agree to this waiver.

By registering for and participating in Stage Ready Academy, I acknowledge and agree to the following:

1. ASSUMPTION OF RISK

I understand that participation includes acting exercises, vocal training, movement, on-camera performance, and use of studio equipment.

I acknowledge that participation may involve risks including:

• Minor physical injury (strain, slips, falls)

• Emotional discomfort associated with performance

• Equipment-related risks (microphones, cables, lighting, recording gear)

• General risks associated with group activities

I voluntarily assume all risks associated with my child’s participation.

2. RELEASE OF LIABILITY

In consideration for participation, I release and discharge MB Studio Productions, Stage Ready Academy, and all owners, instructors, staff, contractors, and affiliates from any and all liability arising from participation, including claims of negligence.

I expressly waive California Civil Code Section 1542, acknowledging that this release applies to unknown claims.

3. MEDICAL AUTHORIZATION

In the event of a medical emergency, I authorize MB Studio Productions to obtain medical treatment for my child if I cannot be reached.

4. BEHAVIOR AGREEMENT

Participants are expected to:

• Follow instructor direction

• Treat others respectfully

• Avoid unsafe or disruptive behavior

• Maintain a positive and supportive environment

Failure to comply may result in removal from the program without refund.

5. MEDIA RELEASE

I understand that the program includes audio and video recording.

I grant permission for Stage Ready Academy to:

• Record my child’s participation

• Provide edited clips to participants

• Use recordings for promotional purposes (unless opted out)

6. DELIVERABLE DISCLAIMER

Participants will receive lightly edited audio/video clips.

No guarantee is made regarding professional production quality beyond basic editing.

7. REFUND POLICY

All registrations are final.

Refunds are only available if cancellation occurs at least 7 days prior to the event.

No refunds for:

• Same-week cancellations

• No-shows

8. ACKNOWLEDGMENT

By checking the agreement box and submitting this form, I confirm:

• I am the parent/legal guardian

• I have read and understand this agreement

• I agree to all terms
